First Visit

1. Your X-Rays if possible (can be emailed prior to appointment)
2. New Patient Registration Form (found in Patient Info)
3. Insurance Card*

We only accept PPO dental plans. We are in In-Network with most insurance companies, but some plans are out of Network. Please check our Insurance tab for more information regarding insurance.

*A Parent or guardian is necessary for all patients under the age of 18.
